The Shahid Shah Show is a weekday radio program on HealthcareNOW Radio. The premise is simple: the healthcare technology industry has no shortage of opinions, but it has a real shortage of people who have actually built, bought, and operated the things they are willing to talk about publicly.

I host the show as a practitioner journalist. I still write code, still lead engineering teams, and still sit on both sides of the buy-build table. That is the whole point: the people worth listening to on digital health are the ones still in the work, not the ones who left it years ago.

Engineering truth, clinical truth, market truth. No hype, no buzzwords.

Each episode is built around someone who has actually shipped something: a founder, a health-system engineer, a product leader, a CIO, a CMIO, or an innovation lead. We talk about what they built, what surprised them, what clinicians rejected, and what they would do differently.

The promise

Three truths, no hype

Every episode is measured against three truths. Engineering truth: does the thing actually work, and can it be maintained? Clinical truth: does it help a clinician or a patient, or does it just add a screen? Market truth: is anyone willing to pay for it, at a price that keeps the company alive?

Most healthcare technology coverage collapses those into one story, usually the market story. The show refuses to. The most interesting failures in this industry happen when the engineering and clinical truths are quietly traded away for the market one, and nobody notices until a patient does.

The three truths

  • Engineering truth: does it work, and can the team keep it working?
  • Clinical truth: does it reduce burden or harm for a real clinician or patient?
  • Market truth: will someone pay for it at a price that sustains the business?

Show format

25–30 minutes of signal, zero noise

Episodes run 25–30 minutes and follow a four-segment structure, designed to keep the conversation practical rather than promotional. The format is deliberate: a founder can pitch anywhere, but they cannot hide from a buyer and a synthesis in the same half hour.

The four segments, in order:

    Reality Check

    A grounded opening on a real issue: why most hospital AI pilots never scale, why interoperability remains unresolved, why EHR add-ons create more complexity than value.

  2. Segment 02

    Builders in the Trenches

    A candid conversation with startup founders, health-system engineers, and product leaders about what they built, what surprised them, and what clinicians rejected.

  3. Segment 03

    The Buyer's Perspective

    CIOs, CMIOs, and innovation leaders on how they evaluate vendors, what signals credibility, and what makes them walk away from a deal.

  4. Segment 04

    Market Synthesis

    A closing segment that distills the episode into action: what to start building, what to stop building, and what buyers should demand before signing.

The recurring segments keep the show honest. Market Mythbusters takes a widely repeated industry claim and tests it against the engineering and clinical truths. Builder's Checklist and Buyer's Checklist close each topic with the questions a builder or a buyer should actually be asking, not the ones the marketing deck wants them to ask.

Topics come from the audience

Episode topics do not come from a content calendar. They come from the community, the founders, engineers, clinicians, and buyers who write in about the problems they are actually wrestling with. If there is a topic you want heard on the show, or a guest whose story should be on the air, bring it to the discussions board.

That is also why the show has a practitioner-journalist frame. I am not interested in hosting a promotional circuit. I am interested in the conversations that only happen when someone still in the work asks the questions a marketer would never ask.
